Job summary

Heart to Heart Hospice in Indiana is seeking a dedicated Hospice Aide (CNA) to provide compassionate, direct patient care under the supervision of the RN, aligned with the Plan of Care.

You will assist with transfers, document care, and communicate changes, while upholding infection control and supportive interactions with patients and families. Benefits include medical, dental, vision, PTO and more for a full-time role.

Qualifications

• High School Diploma preferred or GED.
• Must have an unencumbered CNA license.
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility.
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care.
• EMR experience a plus.
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance.

Responsibilities

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
