Job summary

Guardian Angel Hospice seeks a compassionate Hospice Aide to support patients in-home and in healthcare facilities. You will provide direct care under an RN Case Manager, assisting with bathing, grooming, ADLs, and safety-focused mobility.

You will report observations to the care team and offer emotional support to patients and families. The role requires a current CNA or HHA certification in Indiana, reliable transportation, and flexibility to participate in on-call rotations.

Qualifications

• Current CNA or HHA certification in Indiana required.
• Reliable transportation and a valid driver’s license; flexible for on-call shifts.
• Ability to lift and transfer patients; capable of working in home and facility settings.
• Strong time management and ability to work independently within a team.

Responsibilities

• Provide direct patient care under RN supervision, including bathing, grooming, oral hygiene, skin and nail care.
• Assist patients with activities of daily living to support dignity, safety and comfort.
• Report changes in patient condition to RN Case Manager and care team.
• Safely assist with transfers and mobility using hoyer lifts, gait belts, wheelchairs, and walkers.
• Offer emotional support to patients and caregivers and maintain professional boundaries.

The right candidate will provide essential care and support to our patients and their families, helping with daily living activities and ensuring comfort in alignment with each patient’s individualized Plan of Care.

As a valued member of our care team, the Hospice Aide will:

Provide direct patient care under the supervision of the RN Case Manager, including assistance with bathing, grooming, oral hygiene, skin care, nail care, and other personal needs.

Support patients with activities of daily living (ADLs) in a way that promotes dignity, comfort, and safety.

Report observations and changes in patients’ physical or emotional condition to the RN Case Manager and interdisciplinary team.

Respect professional boundaries while providing care in homes and healthcare facilities.

Safely assist with transfers and mobility, including the use of hoyer lifts, gait belts, wheelchairs, walkers, canes, and crutches.

Offer emotional support and compassionate presence to patients and caregivers.

Requirements:

Current CNA (Certified Nursing Assistant) or HHA (Home Health Aide) certification in the state of Indiana.

Reliable transportation and a valid driver’s license.

Flexibility to participate in the on-call rotation schedule.

Physical ability to perform the duties of the role.

Strong time management skills and the ability to work independently while being part of a supportive team.
