Responsibilities

The Hospice Aide, under the supervision of a registered nurse, provides personal direct patient care services as outlined in the Plan of Care.

• Reports patient condition or environment to the Executive Director or designee after each encounter, following the Care Management process as needed.
• Assists with ADLs such as mobility, transfers, walking, grooming, bathing, dressing, undressing, eating, and toileting during each visit.
• Performs household services essential to the patient's health, helping to prevent or delay institutionalization.
• Participates in QAPI (Quality Assessment & Performance Improvement) activities, reporting problems to the administrator as needed.
• Must have reliable transportation.

Education and Experience

License Requirements

• Completion of a competency evaluation prior to patient care.
• Current CPR certification.
• Valid driver's license, vehicle insurance, and access to reliable transportation.

Skill Requirements

• Mature, able to handle the demands of the position.
• Ability to read, write, and follow directions accurately.
