- Report directly to the Administrator, Executive Director, or Patient Care Manager and function under the supervision of the Registered Nurse and/or Clinical Supervisor.
- Help the patient maintain good personal hygiene.
- Assist in maintaining a safe and healthy environment.
- Prepare and serve light meals.
- Assist in feeding the patient when needed.
- Assist the patient in ambulation and with the safe use of a walker or wheelchair as taught.
- Assist with light housekeeping tasks such as trash and linens.
- Assist with treatments as ordered and permitted by state regulations.
- Promote patient's mental alertness through engagement in activities.
- Provide emotional and psychological support.
- Report changes in patient condition or home situation to the RN and team.
- Practice infection control and document visits accurately.
- Attend staff meetings and participate in quality improvement activities.
Qualifications - About You
- Compassionate, kind, and empathetic to patients and families.
- Comfortable providing care in patients' homes.
- Flexible and composed in crisis situations.
- Certified Nursing Assistant (CNA) license.
- Six months of patient care experience preferred.
- Additional experience in hospital or emergency settings is a plus.
- Valid driver’s license and insurance.
- Current CPR certification.
