Job summary

VitalCaring is seeking a dedicated Hospice Aide/CNA to provide compassionate, in-home end-of-life care. You will assist with daily living activities, support comfort and dignity, and collaborate with RNs, social workers, and chaplains to meet patient needs.

Responsibilities include documenting care, monitoring condition changes, and supporting families with kindness and professionalism in a home setting across visiting locations.

Qualifications

• Completion of state-required Hospice Aide/CNA training.
• Current CPR certification.
• Ability to follow care plans, document clearly, and communicate effectively.
• Comfort working with end-of-life patients and families.
• Strong empathy, compassion and emotional intelligence.

• Assist patients with activities of daily living including bathing, dressing, grooming, toileting and feeding.
• Support comfort through repositioning, mobility assistance and skin care.
• Provide light housekeeping to maintain a safe, comfortable home environment.
• Offer respite support to caregivers and families.
• Observe and report changes in patient condition to the RN/Case Manager.
• Assist with vital signs and basic care tasks as directed.

What You’ll Do

What You Bring

• Completion of state-required Hospice Aide/CNA training (CNA certification if required by state)
• Current CPR certification

• Reliable transportation and ability to travel between patient homes
