Job summary

Longleaf Hospice and Palliative Care is seeking a Hospice Aide to provide compassionate in-home care and assist patients with daily living activities. You will support comfort, safety, and dignity, working under supervision with nursing staff to monitor changes in condition and respond to patient needs.

The role emphasizes personal care, observation, and collaboration to deliver high-quality end-of-life care in the community.

Qualifications

  • Complete state training and competency evaluation or state licensure as per 42 CFR 484.36.
  • Good verbal and written communication and problem solving skills.
  • Valid driver's license and reliable insured vehicle for home visits.
  • Current CPR certification.

Responsibilities

  • Perform personal care as assigned and address patient requests promptly.
  • Ensure safety and use equipment properly.
  • Provide baths, oral hygiene, shampoos, and change linens.
  • Assist with dressing and meals.
  • Observe skin condition and prevent breakdown.
  • Report changes to supervising nurse.
  • Assist with ambulation and exercises.
  • Assist with bedpans and urinals.
  • Assist with light laundry.
  • Follow documentation and care procedures.

As a Hospice Aide, you will perform various services for patients as necessary to meet the patient’s personal needs and to promote comfort through:

  • Performing personal care needs after given a patient's assignment and attending to his/her requests promptly
  • Meeting safety needs of patient and using equipment safely and properly
  • Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linens as often as assigned
  • Assisting in dressing and undressing patients as assigned
  • Assisting with patient meals as assigned
  • Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence
  • Reporting on patient's condition and significant changes to the assigned supervisory nurse
  • Assisting in ambulation and exercise as instructed by the primary care nurse or therapist
  • Offering and assisting with bedpans and urinals
  • Providing assistance as assigned with light laundry needs
  • Performing range of motion and other simple procedures as an extensional therapy service as ordered with appropriate completed/demonstrated skills competency
  • Providing respite for patient's family/caregiver when on-site, as appropriate
  • Keeping patient's living area clean and orderly, as assigned
  • Adhering to the organization's documentation and care procedures and standards of personal and professional conduct
  • Respectful of patient/family/caregiver environment and patient's personal needs

Requirements

  • Successful completion of a state established or other training program that meets the requirements of S 484.36(a) and a competency evaluation program, or state licensure program that meets the requirements of S 484.36(b), or a competency evaluation program or state licensure program that meets the requirements of 42 CFR § 484.36(b)
  • Good verbal and written communication and problem solving skills
  • Responsible valid driver’s license; reliable, insured, automobile for making home visits required
  • Current CPR
