Anchor Health is seeking an After-Hours Hospice LVN/RN to provide skilled nursing care outside standard hours. The role supports continuity of care, symptom management, and emotional support for patients and families after 5:00 PM, with independent work and close teamwork with the interdisciplinary team.

The position requires an RN or LVN license in good standing, hospice or home health experience preferred, a valid driver’s license, and a reliable insured vehicle.

Anchor Health is seeking an After-Hours Hospice LVN/ RN to provide skilled, on-call nursing care outside of standard business hours.

Position Overview

The After-Hours Hospice LVN/RN provides professional nursing care to hospice patients during evenings, nights, and weekends. This role ensures continuity of care, symptom management, and emotional support for patients and families after 5:00 PM.

The nurse works independently while maintaining close communication with the interdisciplinary team, physicians, and leadership to support individualized plans of care.

Schedule

• 5pm-8am
• Weekends and holidays as assigned
• On-call/after-hours coverage

Key Responsibilities

• Respond to after-hours patient calls and urgent care needs
