Anchor Health is seeking an After-Hours Hospice LVN/RN to provide on-call nursing care outside standard business hours in Napa, CA. The role emphasizes independent decision-making, symptom management, and compassionate support for patients and families after 5:00 PM.

The nurse collaborates with the interdisciplinary team, physicians, and leadership to support personalized care plans and regulatory compliance, ensuring seamless continuity of care during evenings, nights, and weekends.

Qualifications

• Active RN or LVN license in good standing.
• Hospice or home health experience preferred.
• Ability to work evenings, nights, and weekends.
• Reliable, insured vehicle and valid driver’s license.

Responsibilities

• Respond to after-hours patient calls and urgent care needs.
• Perform skilled nursing assessments and symptom management.
• Provide emotional support and education to patients and families.
• Update and implement patient plans of care as needed.
• Communicate patient condition changes to the interdisciplinary team.
• Collaborate with physicians and caregivers regarding patient needs.
• Complete accurate and timely clinical documentation.
• Uphold hospice philosophy, policies, and regulatory standards.

Anchor Health is seeking an After-Hours Hospice LVN/ RN to provide skilled, on-call nursing care outside of standard business hours.

Position Overview

The After-Hours Hospice LVN/RN provides professional nursing care to hospice patients during evenings, nights, and weekends. This role ensures continuity of care, symptom management, and emotional support for patients and families after 5:00 PM.

The nurse works independently while maintaining close communication with the interdisciplinary team, physicians, and leadership to support individualized plans of care.

Schedule

• 5pm-8am
• Weekends and holidays as assigned
• On-call/after-hours coverage

Key Responsibilities

• Respond to after-hours patient calls and urgent care needs
• Perform skilled nursing assessments and symptom management
• Provide emotional support and education to patients and families
