Job summary

Anchor Health is seeking an After-Hours Hospice LVN/RN to provide skilled, on-call nursing care after 5 PM, evenings, nights and weekends.

You will work independently while coordinating with the interdisciplinary team to support individualized plans of care and ensure seamless symptom management and compassionate support for patients and families.

The role requires RN or LVN licensure in good standing; hospice or home health experience is preferred; on-call schedule expectations are included.

Qualifications

• Current RN or LVN license in good standing.
• Hospice or home health experience preferred.
• Strong clinical judgment; able to work independently.
• Excellent communication and problem-solving skills.
• Valid driver’s license and reliable, insured vehicle.

Responsibilities

• Respond to after-hours patient calls and urgent care needs.
• Perform skilled nursing assessments and symptom management.
• Provide emotional support and education to patients and families.
• Implement and update patient plans of care as needed.

• Collaborate with physicians and caregivers regarding patient needs.

Anchor Health is seeking an After-Hours Hospice LVN/ RN to provide skilled, on-call nursing care outside of standard business hours.

Position Overview

The After-Hours Hospice LVN/RN provides professional nursing care to hospice patients during evenings, nights, and weekends. This role ensures continuity of care, symptom management, and emotional support for patients and families after 5:00 PM.

The nurse works independently while maintaining close communication with the interdisciplinary team, physicians, and leadership to support individualized plans of care.

Schedule

• 5pm-8am
• Weekends and holidays as assigned
• On-call/after-hours coverage

Key Responsibilities
