Job summary

Management Performance Associates is seeking a Hospice Admissions RN for the Kansas City metro area. This role coordinates new patient admissions, conducts comprehensive assessments, educates patients and families, and collaborates with the care team to ensure a smooth transition into hospice services.

The position is full-time, within a 50-mile radius of the office, with a Tuesday–Saturday schedule from 10am to 7pm.

Qualifications

• Current Registered Nurse license valid for practice in Missouri.
• Current driver's license.
• Current CPR certification.
• Two years of Registered Nurse experience in a clinical care setting.
• One year of full-time hospice experience.
• Reliable transportation and valid automobile liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support seamless communication and continuity of care.
• Educate patients and families regarding condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
• Perform other duties as assigned.
