Hearth Hospice is hiring a Full Time Compassionate Resource Nurse (RN) to deliver patient care in and around Fort Oglethorpe, GA. The role partners with intake, physicians, and the Hospice Medical Director to assess referrals and support admissions.

Responsibilities include in-home or facility visits, nursing care, family education, and on-call duties. Requires RN license, CPR, 1+ year acute care experience, and EMR proficiency.

Qualifikationen

• Bachelor's degree in nursing or health care required.
• Must hold current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care.
• Hospice/home health experience preferred but not required.
• Must be proficient with computers and EMR systems.
• CPR certification is required.
• Valid driver's license, vehicle insurance, and reliable transportation.

Aufgaben

• Review clinical referral information and assess eligibility for hospice services.
• Provide pre-assessment, admission services, and in-home or facility visits as needed.
• Coordinate with Intake, referring physician, and Hospice Medical Director on referrals.
• Deliver nursing care and education to patients and families; support plan of care goals.
• Participate in on-call duties as assigned.
