Job summary

Three Oaks Hospice in Georgia is seeking a compassionate Registered Nurse to administer skilled nursing care in patients' homes. You will coordinate with the interdisciplinary team, assess needs, and implement care plans for patients of all ages requiring hospice services.

Must have an RN license, CPR, and 2 years of clinical RN experience; hospice/home health background preferred. This full-time role covers Powder Springs, Dallas, Hiram, Austell, and Mableton, with on-call duties and a sign-on

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver's license.
• Two years’ RN experience in clinical care; hospice/home health preferred.
• CPR certification required.
• Proficient in computer use; MS Office experience preferred.
• Strong interpersonal, communication, and organizational skills.

Responsibilities

• Administer skilled nursing care in patients' homes and coordinate with the IDT.
• Perform initial and ongoing assessments of patient needs and ADLs.
• Develop and revise the Plan of Care with the care team.
• Educate patients and families about disease processes and end-of-life care.
• Provide on-call coverage and support to patients and families.
• Documentation of progress notes and care coordination activities.
• Communicate changes in patient status to physicians and team.
