Job summary

Hospice of Marion County, part of Empath Health, seeks an Admissions RN to guide patients and families through the admissions process in homes, facilities, and hospitals across Marion County.

PRN schedule with minimum 64 hours per month; hospice experience preferred. Join a compassionate team delivering Full Life Care and support to families at a challenging time.

Qualifications

• Hospice experience strongly preferred.
• Education: AA degree or equivalent; two-year college or technical school, or related experience.
• Licensure: Florida RN license and Florida driver’s license.

Responsibilities

• Assess eligibility for Empath Health Network of Care as a first responder.
• Advocate for optimal access to the right care in any setting.
• Complete admission process based on care program specifics and patient needs.
• Coordinate with the Professional Liaison to deliver educational/in-service tools in hospitals.
• Educate community referral sources about Empath Health programs.

Schedule: PRN, minimum of 64 hours a month availability. Hospice experience required.

What you'll Do

• The Admissions RN is a first responder who assesses eligibility for Empath Health Network of Care
• Advocating for optimal access to the right care at the right time in any care setting.
• The Admissions RN completes the admission process based on care program specifics, guidelines, and patient-specific needs.
• Coordinate and assist with the Professional Liaison to deliver educational, in-service and outreach tools in the hospital.
• Works directly with community referral sources to educate regarding Empath Health programs and services.
• Complies with RN On Call scheduling needs as well as vendor credentialing process.

What You'll Need

• Hospice experience strongly preferred
• Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
• Licensure:
  • Valid Florida license as a Registered Nurse
  • Valid Florida State Driver’s License
• For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
