Job summary

Hospice of Marion County, a member of Empath Health, is seeking an RN Case Manager to join the Care team providing direct patient care in facilities in Marion County. Schedule: Monday - Friday, daytime hours.

Qualifications

  • Associate’s degree (A.A.) or equivalent from two-year college or technical school.
  • Minimum of one year related experience and/or training; or equivalent combination of education and experience.
  • Valid Florida license as a Registered Nurse
  • Valid Florida State Driver’s License
  • Field-based roles require reliable transportation and proof of automobile insurance.

Responsibilities

  • Obtain patient history, perform age-specific assessments, and initiate referrals following guidelines.
  • Provide continuity of care, develop a Plan of Care with patient/family and IDT, and update it as needed.
  • Assist with closure contact and support after a patient’s death in collaboration with the IDT.
  • Direct and supervise care by LPN and Home Health Aide, with biweekly evaluations.
  • Administer and document medications as prescribed and provide patient/family teaching.

Hospice of Marion, a member of Empath Health, is seeking a caring and motivated RN Case Manager to join the Care team providing direct patient care in facilities in Marion County.

Schedule: Monday - Friday, daytime hours.

What You’ll Do
  • Demonstrates the ability to obtain patient history, perform an age-specific physical, psychosocial and spiritual assessment and initiates appropriate referral, intervention and collaboration between the patient, family, Interdisciplinary team (IDT), physician and partnering facility (as applicable) following policy and practice guidelines.
  • As RN Case Manager provides continuity of care, care coordination, identifies patient/family goals, develops a Plan of Care with patient, family, IDT, attending and physician, partnering facility and evaluates progress towards goals. The Plan of Care will be updated according to policy/guidelines and as needs change.
  • Following a patient’s death, assists with closure contact and support in collaboration with IDT.
  • Directs and supervises the care provided by the LPN and Home Health Aide following agency policy. Evaluates services provided by the Home Health Aide every two weeks with periodic on-site supervisory visits.
  • Demonstrates the ability to administer, monitor and document medications as prescribed by attending physician. Uses age‑specific care practices and provide appropriate patient/family teaching.

What You’ll Need
  • Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school; Minimum of one year related experience and/or training; or equivalent combination of education and experience.
  • Licensure:
    • Valid Florida license as a Registered Nurse
    • Valid Florida State Driver’s License
  • Prefer one (1) year experience in an acute or post acute care setting
  • Preferred experience caring for terminal patients and others with life threatening illnesses.
  • For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
