Job summary

Midland Care Connection in Topeka, KS is seeking an experienced Hospice Admissions Nurse to guide patients and families into hospice care with clarity and compassion. You will assess eligibility, coordinate plans with the hospice team, and ensure smooth transitions from home or facility.

The role requires an active RN license (ADN/BSN), strong assessment and documentation skills, and reliable transportation.

Qualifications

• Active RN license.
• Graduate of an accredited nursing program (ADN or BSN).
• Valid driver’s license with reliable transportation.
• Current CPR certification.
• Strong clinical assessment and documentation skills.
• Ability to work independently and manage time effectively.
• Excellent communication and interpersonal skills.
• Ability to provide compassionate care to patients and families.

Responsibilities

• Complete initial assessment by reviewing patient medical history and evaluating symptoms to determine hospice eligibility.
• Explain hospice philosophy, services, and care plan to patient and family; answer questions and obtain consents.
• Collaborate with physicians and hospice team to develop individualized plan of care.
• Ensure medications and equipment are arranged for transitions.
• Support families during the transition to hospice care with empathy and guidance.
