Job summary

Suncrest Hospice in the Richmond area seeks a dedicated Registered Nurse to provide in-home and facility-based care as part of a multidisciplinary team. You will assess, plan, and deliver patient-centered hospice services across homes, SNFs and assisted living facilities.

You will collaborate with nurses, social workers, chaplains and other staff, handling documentation, visits, and meetings while maintaining compliance with hospice regulations. Travel within the service area is required.

Qualifications

• Current RN license in good standing in state(s) of practice.
• Ability to collaborate with multidisciplinary care teams.
• Experience in direct patient care and coordination.
• Proficient in written and verbal communication; able to manage multiple communications.
• Strong time management for visits, documentation and meetings.
• Experience learning electronic medical records systems.
• Willing to travel within service area with reliable transportation.

Responsibilities

• Provide care for a group of patients visiting homes and facilities.
• Contribute to interdisciplinary team with psychosocial assessments and interventions.
• Work in Case Management, Admissions, or Resource coordination.
• Maintain documentation and coordinate patient care meetings.
