Job summary

VITAS Healthcare is seeking a compassionate Hospice Admissions Nurse to bridge hospital-based care with hospice services, guiding patients and families through this critical transition.

You will educate families about Medicare benefits, collaborate with physicians and care teams, and manage admissions with empathy and clinical excellence to ensure a smooth, patient-centered experience.

Qualifications

• RN with a valid state license and clinical nursing experience.
• Minimum 2 years of nursing experience (hospice, palliative, ER, ICU, home health or case management preferred).
• Strong clinical assessment and critical thinking skills.
• Compassionate communicator with excellent interpersonal skills.
• Active listener who builds trust and rapport.
• Patient-centered advocate committed to dignity and comfort.

Responsibilities

• Conduct comprehensive clinical assessments to determine hospice eligibility.
• Educate patients and families about hospice services and Medicare benefits.
• Collaborate with physicians, case managers, and interdisciplinary teams.
• Provide emotional support and clinical guidance during decision-making process.
• Initiate and complete the admission process, including documentation and coordination.
