Job summary

VITAS Healthcare in Florida is seeking a Hospice Admissions Nurse to serve as a compassionate bridge between hospital or community care and the comfort of hospice. You will guide patients and families through the transition with clarity, empathy, and confidence.

You’ll conduct comprehensive clinical assessments to determine hospice eligibility, educate on services and Medicare benefits, and collaborate with physicians and interdisciplinary teams to coordinate admissions and care planning.

Qualifications

• Registered Nurse with a valid state license.
• Minimum 2 years nursing experience (hospice, palliative, ER, ICU, home health or case management preferred).
• Strong clinical assessment and critical thinking skills.
• Compassionate communicator with excellent interpersonal skills.
• Active listener who builds trust and rapport.
• Reliable professional with strong follow-through and autonomy.

Responsibilities
