Job summary

As an RN with a valid license and at least 2 years of nursing experience, you will assess eligibility, educate families on Medicare benefits, and collaborate with physicians and interdisciplinary teams. This role supports a meaningful calling and a structured schedule.

Qualifications

• Registered Nurse with a valid state license.
• Minimum 2 years of nursing experience (hospice, palliative, ER, ICU, home health or case management).

Responsibilities

• Conduct comprehensive clinical assessments to determine hospice eligibility.

• Provide emotional support and clinical guidance during the decision-making process.
• Initiate and complete the admission process, including documentation and coordination.
