Job summary

Qualifications

• Active RN license in CT or ability to obtain CT licensure.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes and revises the total plan of care in collaboration with the care team.
• Communicates patient progress to the clinical manager and physician.
• Maintains individualized patient care progress records.
• Educates families on patient condition and care needs.
• Performs on-call responsibilities as assigned.
• Performs other duties as assigned.
