Job summary

Visiting Nurse Association of Texas is seeking a compassionate Registered Nurse (RN) Admissions to be the first clinical contact, performing initial assessments and coordinating in-home hospice care from day one.

Qualifications

• Must have strong interpersonal skills and compassionate bedside manner.
• Solid knowledge of general nursing standards and applicable hospice regulations.
• Experience in hospice or home health settings is preferred and beneficial.

Responsibilities

• Assess patients in their home setting and document findings.
• Develop and document plans of care aligned with initial assessments and infection control measures.
• Coordinate with the IDT and clinicians to ensure seamless transitions into hospice services.
• Maintain timely, accurate clinical documentation compliant with regulations.
• Meet productivity targets (2-3 admissions per day) and participate in on-call rotations.
