Qualifications

• Must be a Registered Nurse with a valid state license.
• Minimum 2 years nursing experience; hospice/palliative preferred.

Responsibilities

• Conduct comprehensive clinical assessments to determine hospice eligibility

• Collaborate with physicians, case managers, and interdisciplinary teams
• Provide emotional support and clinical guidance during decision-making
• Initiate and complete admission process, including documentation and coordination
