Job summary

HopeHealth is seeking a dedicated Hospice Admissions Registered Nurse (RN) to provide skilled assessments and admissions for patients in home and long-term care settings in Massachusetts. This full-time role includes 40 hours per week with a 1:00pm-9:30pm shift and rotating weekends.

A $2000 sign-on bonus is available. As the Admissions RN, you will evaluate patient eligibility, document Local Coverage Determination, and coordinate with the interdisciplinary team to develop a personalized plan

Qualifications

• RN licensed in MA and RI.
• BSN preferred.
• 1–3 years of hospice or acute care experience.
• Excellent nursing assessment and communication skills.

Responsibilities

• Perform initial patient/family assessments to determine eligibility for hospice services.
• Complete hospice admissions in compliance with Medicare/Medicaid and state regulations.
• Document in EMR at point of care and ensure timely charting.
• Create individualized plans of care with Local Coverage Determination documentation.
• Use an interdisciplinary team approach to develop care plans and initiate referrals as needed.
• Participate in weekly hospice IDG meetings and weekend coverage as required.
• Coordinate payer information and verify insurance coverage.
• Support field evaluations/admissions and GIP coverage as needed.
