Job summary

Compassus is seeking a Hospice Admission Registered Nurse (RN) to assess patients for hospice eligibility and coordinate care in a home-based setting in Georgia. You will collaborate with the interdisciplinary team to establish initial plans of care and support families through the admission process.

The role requires an active RN license in the state, 2 years of RN experience (hospice preferred), a valid driver’s license, CPR/BCLS certification, and a passion for compassionate, patient-centered

Qualifications

• RN license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Current driver's license and CPR/BCLS certification.

Responsibilities

• Assess patients for hospice eligibility and discuss services with families.
• Coordinate care with the Inter-Disciplinary Team to establish initial plan.
• Support patients in maximizing comfort and quality of life at home.
