Job summary

Anchor Hospice is seeking an Admissions RN in Walnut Creek, CA to present hospice philosophy and services to patients and families, guiding appropriate admissions. Responsibilities

• Present ANCHOR Hospice services and philosophy to patients and families.
• Assist in information gathering, analysis, education and form completion during admissions.
• Evaluate patients for hospice appropriateness through chart review when requested.
• Build effective referral source relationships to support high-quality responsiveness.
• Maintain clinical and professional competence.
• Develop internal relationships with admissions and patient care teams.
• Educate patients and families about hospice and palliative care and address questions.
