The Elizabeth Hospice is seeking a qualified Hospice Nurse to perform initial patient assessments and develop comprehensive plans of care upon admission across patient lifespans. You will collaborate with the IDG to ensure seamless care and educate communities about available services.

Position requires travel within North San Diego County and South Riverside, CA, with daily 12-hour shifts (4 on) and hourly pay between $45 and $61 depending on experience.

Qualifications

• Associate’s Degree required; Bachelor’s Degree preferred.
• Current CA RN license.
• Current CPR certification and negative TB skin test.
• Bilingual (Spanish/English) preferred.
• Computer skills (word processing).
• Current California driver’s license and proof of auto insurance.
• Experience as Hospice nurse preferred.

Responsabilités

• Performs visits to assess and evaluate patients’ medical conditions and plan of care.
• Advocates for patients and families and incorporates preferences.
• Manages time to meet admissions expectations.
• Directs care provided by LVN and HA per plan of care.
• Supervises services provided by LVN and HA.
• Assesses and coordinates admissions to hospice.
• Coordinates with attending MD and Hospice Medical Director.
• Communicates initial assessment, admission, and plan of care to the IDG.
