Job summary

AccentCare, Inc. is seeking a Clinical Liaison VIPC, Inpatient Hospice Admissions RN to support Memorial Hermann Katy on weekends (10a-10p).

The role focuses on identifying patient needs, coordinating admissions, and ensuring timely transitions to hospice services with a trusted clinical resource for education and eligibility. You will interact with physicians, hospital staff, and care teams, advocating for patients and families while upholding ethics and quality in a hospital-based hospice

Qualifications

• Active RN license and valid driver’s license with insured vehicle.
• Minimum 1 year RN experience with knowledge of home health regulations and Medicare requirements.
• Medical sales or clinical outreach experience preferred.
• Strong communication and relationship-building skills; ability to work independently in a hospital setting.

Responsibilities

• Build and maintain trusted relationships with physicians, hospital staff, and care teams.
• Serve as the primary clinical resource for hospice education, eligibility, and appropriateness.
• Conduct patient assessments and coordinate the admission process.
• Partner with Admissions, Clinical, and Care Teams for smooth transitions.
• Advocate for patients and families during goals-of-care conversations.
• Participate in interdisciplinary planning, training, and outreach.
• Uphold ethics, quality, and continuous learning.
• Meet expectations for response times, referral coordination, and communication.
