Job summary

True North Consultants, Inc. is seeking a dedicated Hospice Admissions RN for the Kansas City Metro area (east) to join our team on a full-time basis.

The nurse will coordinate new patient admissions, perform comprehensive assessments, educate patients and families, and collaborate with the interdisciplinary care team to ensure a smooth transition into hospice services. This role requires Missouri RN licensure, CPR certification, and hospice experience; schedule is Tuesday–Saturday with a

Qualifications

• Currently licensed as a Registered Nurse in Missouri.
• Valid driver's license.
• CPR certification current.
• Two years of RN experience in a clinical care setting.
• One year of full-time hospice experience.
• Reliable transportation and auto liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support seamless communication and continuity of care.
• Educate patients and families regarding condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
• Perform other duties as assigned.
