Job summary

LanceSoft is seeking a Hospice Admissions Registered Nurse (RN) for a full-time permanent role in Denton County, TX. The RN will perform admissions assessments, determine eligibility, and develop patient-centered care plans for hospice patients.

Ideal candidates hold an active Texas RN license, have at least 1 year of hospice or senior care experience, and possess BLS certification. On-site schedule is Monday–Friday with 8:00 AM–5:00 PM and on-call rotation.

Qualifications

• Active Texas RN license is required.
• Minimum 1 year hospice or senior care experience.
• BLS certification is required.
• Valid driver's license with reliable transportation.
• Strong communication and patient assessment skills are essential.

Responsibilities

• Complete hospice admission assessments.
• Determine hospice eligibility.
• Develop individualized care plans.
• Educate and support patients and families.
• Coordinate with the interdisciplinary care team.
• Maintain accurate clinical documentation.
