CONSUMER DIRECTED SERVICES INC. is seeking a field-based Hospice RN Admissions to conduct in-home admission assessments and start-of-care visits. You will evaluate hospice eligibility and educate patients and families about available services, forming the initial care plan based on clinical findings.

Collaborating with physicians and the interdisciplinary team, you will ensure continuity of the plan of care and support safe, high-quality home-based care for elderly and chronically ill patients.
