Job summary

Novant Health's Hospice Liaison Nurse role at Rowan Medical Center is a hospital-based position that coordinates referrals, conducts clinical assessments, and facilitates admissions to hospice services. You will work closely with physicians, case managers, discharge planners, social workers, and interdisciplinary teams to ensure timely access and smooth transitions.

Ideal candidates hold RN licensure, a BSN is preferred, and have at least three years in home care or hospice.

Qualifications

• RN licensure in appropriate state.
• BSN preferred.
• Minimum 3 years of home care or hospice experience.
• Must be able to travel between different locations as assigned.

Responsibilities

• Process hospice referrals and coordinate admission scheduling.
• Conduct comprehensive clinical assessments to determine eligibility.
• Facilitate hospice admissions and transitions from hospitals to hospice.
• Educate patients, families, and referral sources on hospice services and eligibility.
• Collaborate with physicians, case managers, social workers, discharge planners, and interdisciplinary teams.
• Support discharge planning and remove barriers to timely hospice placement.
• Build and maintain relationships with hospital staff and community partners.
• Ensure compliance with hospice regulations and organizational policies.
• Advocate for patients and families to promote comfort and dignity.
• Coordinate care to ensure excellent patient and family experiences.
