Socket.dev is seeking a Hospice Admissions Coordinator in Arizona to lead the admissions process from inquiry to enrollment, coordinating referrals from hospitals and care settings. The Hospice Admissions Coordinator is responsible for leading and coordinating all aspects of the admissions process, ensuring patients and families experience a smooth, compassionate, and timely transition into hospice care. This role manages patient referrals, oversees the admission process from initial inquiry to enrollment, and ensures accurate and timely documentation in compliance with local, state, and federal regulations. A key part of this role includes engaging with referral sources and guiding prospective patients and families to hospice admission when appropriate, helping them understand the benefits and support available. The Admissions Coordinator provides leadership to the Support Services staff, ensuring proper maintenance and compliance of all records. Serving as a liaison among patients, families, referral sources, and the interdisciplinary team, this position fosters strong community and professional relationships to support program growth and uphold the highest standards of care and service.
