Job Description:
We are seeking an engaging, positive, organized, experienced Care Manager to join our team as we work together to meet the homecare needs of persons aging in their home and community.This individual should havean excellent working knowledge of the population we serve coupled with experience recruiting, retaining and supervising staff.
In this role, theCare Managerwill provide assessment and service-coordination to meet at-home needs for eligible consumers,conduct home visits with new and existing consumers, provide consistent outreach efforts to new and existing referral sources, excel in a fast-paced environment, be a solutions driven individual and be passionate about “doing whatever it takes” to fulfill our compelling mission.
Qualifications:
Bachelor’s degree in Human Services or a related field
Two (2)+ years working with or having a working knowledge of elderly, chronically ill or disabled population
Valid driver’s license, automotive insurance and reliable transportation
Experience Supervising staff strongly preferred
Hours :Full-time, 40 hours, Monday - Friday withoccasionalon call hours.
Compensation: Base pay starts at $58,656 annually
