Salary Range: $24.52 - $24.52 Hourly

The Homeless Prevention Case Manager will provide support services to participants of MBI’s Homelessness Prevention Services Program. They will offer assistance to at-risk families to lead healthy lives through services that include case management, community referral linkages, rental/utility assistance, substance abuse referrals, mental health service referrals, short-term life skills building, and housing needs. This role reports directly to the Prevention Program Manager. Applicants must be comfortable working as a team member and demonstrate a strong commitment to serving at-risk communities in a professional environment.

Essential Duties and Responsibilities

  • Provide administrative and program support in the following areas: intake, client wellness, community building, documentation, and special projects.
  • Work with individuals to set client goals and implement strategies to meet those goals. Track and report on progress regularly.
  • Develop comprehensive case management plans for participating families.
  • Assist in processing lease agreements with landlords, families, and contractors as needed.
  • Actively network with community service providers and partner agencies to locate programs and resources to support families.
  • Assess client needs and make referrals as appropriate.
  • Input client data into the Homeless Management Information System (HMIS). Generate client reports and issue financial assistance checks as necessary.
  • Establish and maintain weekly or bi-weekly meetings based on client assessments.
  • Attend all company-required training sessions.
  • Perform other relevant duties as assigned.
Qualifications
  • Bachelor’s degree in social work or 3-5 years of experience working within the homeless continuums
  • Crisis management experience is a plus.
  • Excellent written and verbal communication skills.
  • Exceptional ability to multi-task with strong organizational skills.
  • Proven track record of success in case management.
  • Prior experience and understanding of the causes of homelessness.
  • Experience working with landlords, local rental markets, and/or housing sales.
  • Ability to work effectively in a team-oriented environment.
  • Ability to work with and serve a diverse population, demonstrating respect and appreciation for diversity based on race, class, ethnicity, religion, gender, sexual orientation, and gender presentation.
  • Ability to work effectively with a highly diverse group of clients, co-workers, and colleagues.
