Job summary

Trinity Health at Home is seeking a Home Care RN to provide compassionate, one-to-one nursing care in patients' homes in Newark, DE. A $10K signing bonus applies to eligible external candidates.

The RN will perform initial assessments, develop care plans with the Population Health Case Manager, and supervise Home Healthcare Aides and LPNs while delivering high-quality, patient-centered care. Join a supportive team and a mission-driven organization committed to excellence.

Qualifications

• Graduate of an approved nursing education program
• Licensure as a Registered Nurse in Delaware
• At least 1 year of professional nursing experience
• Acute Care or Home Care experience preferred
• Must have current Driver’s license and reliable transportation
• Ability to work a weekend rotation once a month
• Ability to work 2 holidays yearly (1 Summer & 1 Winter)
• No on call duties

Responsibilities

• Provide primary nursing care to home-based patients as ordered by the physician.
• Conduct initial assessment and care plan development in collaboration with the Population Health Case Manager.
• Provide functional supervision to Home Healthcare Aides and LPNs as needed.
• Deliver therapeutic intervention, overall care management and achievement of patient satisfaction.
• Aim for top decile client outcomes and patient satisfaction.
• Collaborate with the care team to coordinate services.

Minimum qualifications
