Job summary

St. Mary Home Care, an agency of Trinity Health At Home, has a full-time Home Health Registered Nurse - Case Manager opening serving Bucks County, PA. You will provide in-home skilled nursing care, develop personalized care plans, administer medications and support recovery and independence.

We offer a signing bonus of up to $10,000, day-one health benefits, competitive pay and a supportive leadership team. Weekend rotations and reliable transportation are required for this role.

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the state of PA.
• Two years experience as a professional care nurse with home care experience preferred.
• Must have current Driver’s license and reliable transportation.
• Ability to work a weekend rotation once monthly & 2 holidays annually.

Responsibilities

• Provide in-home skilled nursing care and develop personalized care plans.
• Administer medications and support recovery and independence.
• Deliver care in a home health setting with independence and patient-centered focus.
