Job summary

Intermountain Health in Ogden, Utah seeks a licensed Physical Therapist to screen, evaluate, and treat injuries and disabilities using standard physical therapy practices in a homecare setting.

You will collaborate with care teams, educate patients and families, supervise staff, and participate in quality improvement initiatives while delivering patient-centered care and maintaining accurate records. Scheduled hours total 40 per week.

Qualifications

• Current Physical Therapist license in states where you work.
• BLS for healthcare providers.
• Basic computer skills.
• Exceptional interpersonal and communication skills.
• Possess skill sets and experience with target population of job setting.
• Current driver’s license, reliable transportation, and acceptable driving record.

Responsibilities

• Provides skilled physical therapy services and develops care plans based on patient assessments.
• Documents patient records and billing accurately and promptly.
• Communicates with patients, families, physicians, and care teams about patient needs and goals.
• Supervises assistants, aides, and office staff to ensure high-quality care.
• Participates in continuing education and quality improvement initiatives.
• Contributes to program development and marketing strategies for the physical therapy program.
