Chapters Health System is seeking a dedicated homecare professional to develop and implement services for frail elderly participants in Florida. You will assess needs, coordinate with IDT members, and ensure care plans are followed while maintaining accurate documentation.
Responsibilities include coordinating outside agency staff, updating patient data, and monitoring service quality to meet participant needs. A valid Florida driver’s license and BLS certification are required.

Education and Experience High school diploma or general education degree (GED) is required. Must have one or more year’s experience with a frail or elderly population. Experience working with outside agencies and customer service experience is preferred.
Certificates, Licenses, Registrations Must possess a valid Florida driver’s license, current auto insurance, and reliable transportation. Active BLS for healthcare professionals from the American Heart Association or Red Cross.
Mathematical Skills Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Reasoning Ability Ability to solve problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Ability to communicate, concentrate and document effectively and accurately.
Language Skills Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.

Pay Range: $16.96 - $25.24
