Job summary

VNA Health Group is seeking a full-time Licensed Practical Nurse to provide quality homecare nursing services under the direction of a Registered Nurse. You’ll deliver care plans, educate patients, and collaborate with a multidisciplinary team to ensure safety and compassion in patients’ homes.

A NJ LPN license and 2+ years of relevant experience are required, plus a valid driver’s license and willingness to travel.

Qualifications

• Graduate of an NLN-accredited LPN program with an active NJ LPN license.
• 2+ years of med/surg or acute care experience (homecare/hospice a plus).
• Strong communication, time management, and tech skills.
• A valid driver’s license and willingness to travel.

Responsibilities

• Bringing care plans to life under the guidance of an RN.
• Teaching and supporting patients with preventative and recovery care.
• Ensuring services meet high standards of quality and compassion.
• Collaborating with doctors, nurses, and other care professionals.
• Maintaining patient safety, documentation, and infection control standards.
• Helping patients regain independence and dignity.
