Qualifications

• Must hold an accredited Master’s or Doctoral degree in Nursing or a Physician Assistant program.
• Current certification as an Advanced Practice Registered Nurse (APRN) or PA is required.
• Licensed to practice as a Nurse Practitioner or Physician Assistant in Kentucky.

Responsibilities

• Provide primary care to a patient panel in homes, assisted living facilities, and personal care facilities.
• Plan, direct, and evaluate patient care; treat chronic and urgent conditions; perform selected procedures.
• Write prescriptions and coordinate with supervising physicians and care teams.
• Develop and communicate individualized care plans; contribute to clinical protocol development and quality improvement.
