Essen Health Care in New York seeks a Physician Assistant (PA) specializing in house calls to deliver high-quality medical care to patients in their homes, focusing on those with chronic illness, limited mobility, or barriers to traditional care. You will work with a multidisciplinary team, perform home assessments, develop treatment plans, coordinate care, and educate patients and families.

Travel is required within the service area, and bilingual Spanish is a plus.

Competenze

• Master’s degree required.
• Active New York license as a Physician Assistant.
• New York Controlled Substances Registration.
• Graduate of an approved PA program and post-graduate training in surgery or equivalent work experience.
• NCCPA certification.
• Specialty in Emergency Medicine / Family Practice / Acute Care.
• BLS/ACLS certifications preferred.
• Spanish bilingual is a plus.

Mansioni

• Perform home-based patient assessments, including health status, examinations, histories, and medication reviews.
• Diagnose conditions and develop personalized treatment plans; prescribe medications as appropriate.
• Coordinate care with physicians, nurses, and other team members for continuity of care.
• Educate patients and families on disease management, preventive care, and lifestyle changes.
• Document patient encounters and treatment plans in electronic health records.
• Schedule follow-up visits and adjust treatment plans as needed.
• Assess urgent medical needs during home visits and coordinate referrals.
• Engage in community outreach to promote home healthcare services.
