Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide therapy services for Hospice Patients in a home setting in Inverness, FL. This PRN role offers flexible assignments and competitive compensation, with clinical autonomy and a supportive team environment.

Responsibilities include evaluating and treating residents in home or assisted living settings, creating individualized treatment plans, collaborating with the care team, and maintaining EMR documentation.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and or Home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
