Sobe Innovative Rehab in Inverness, FL is seeking a licensed Physical Therapist to provide hospice therapy in a home setting on a PRN basis. The role emphasizes compassionate, evidence-based care and maintaining patient independence.

You will evaluate residents, develop individualized plans, collaborate with physicians and families, and maintain EMR documentation. Active Florida PT license required; new grads welcome; bachelor's, master's or DPT degree accepted.

Qualifications

• PT license active in Florida.
• Degree in Physical Therapy from an accredited program.
• New graduates and experienced therapists are welcome.

Responsibilities

• Evaluate and treat residents in an assisted living community and/or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
