Job summary

Aging Well Health Care, LLC is seeking a licensed Physical Therapy Assistant (PTA) to administer therapy in patients’ homes in Missouri. The role follows physician orders and the PT plan of care, under supervision.

The schedule is flexible, typically 2–5 days weekly, with hours often 3PM–7PM as patients return home from day programs. The PTA will collaborate with PT and health care staff, perform required documentation, and educate families on home programs and daily living activities.

Qualifications

• Graduate from a two-year college PTA program approved by the APTA.
• Currently licensed in the state(s) where practicing.
• Two years of experience preferred.
• Ability to exercise initiative and independent judgment.
• Acceptance of philosophy and goals of Agency.

Responsibilities

• Provides physician-prescribed physical therapy under a PT plan of care.
• Improves or minimizes residual physical disabilities and returns patients to optimal function.
• Prepares and updates medical records, progress notes, and care plans.
• Educates patients and families in home programs andactivities of daily living.
• Participates in care conferences, in-service programs, and performance improvement activities.
