Job summary

SSM Health Rehabilitation Network – Home Health is seeking a dedicated Physical Therapist Assistant (PTA) to join our home care team serving the Central Missouri region. You will work under the supervising therapist to collect data, implement the plan of care, and progress patients in their home environment.

The PTA will coordinate care with other clinicians, provide patient education for home programs, and assist with discharge planning and equipment needs.

Qualifications

• Current state licensure or eligibility for state licensure as a Physical Therapy Assistant.
• Certified BLS or completion within first 90 days of employment.
• One or more years of clinical experience.
• Valid state Driver’s License.

Responsibilities

• Assist the therapist with data collection and initial evaluations as directed.
• Help implement therapy plans and perform delegated procedures.
• Progress treatment as the patient’s status changes under supervision.
• Identify and report changes in patient status to the supervising therapist.
• Coordinate treatment programs with other clinical services and community resources.
• Provide patient education for home programs for safe discharge.
• Prepare for patient discharge including equipment orders/adaptations and recommendations.
• Assist with directing functional training classes under the supervising therapist.
• Document daily notes and charges in the patient’s medical record as required.
