Responsibilities

• Visit health plan members at home for in‑home health evaluations.
• Build trusted relationships and help bridge gaps in care.
• Travel locally within licensed states as needed and conduct virtual visits when required.
• Utilize iPad and the clinical workflow during evaluations.
