Qualifications

• Active, unrestricted licensure to practice in Illinois.

• Access to reliable transportation required for home visits.

Responsibilities

• Conduct comprehensive in-home assessments, including medical history and physical exams.
• Document diagnoses for treatment planning and communicate findings effectively.
• Educate members on health processes and involve them in care management.

Skills

Clinical experience (family, geriatric, or home health)

Proficiency with electronic medical records

Communication skills with geriatric or Medicare populations

Education

Active ANCC or AANP certification for NPs
