Job summary

AbsoluteCARE Medical Center & Pharmacy is seeking a Community Based Clinician to deliver primary and urgent care in patients' homes in Baltimore, MD. This role focuses on care transitions after hospital discharge, collaborating with PCPs and care managers to ensure seamless, patient-centered care.

The clinician will manage a small panel of homebound patients, coordinate with ICT members, and educate patients on chronic disease management and medication adherence.

Qualifications

• CRNP in Adult Medicine with Acute Care experience is required.
• ACLS certified.
• Willingness to perform home visits and coordinate care transitions.

Responsibilities

• Provide community-based medical and care-coordination services for recent hospital discharges.
• Manage a small panel of homebound members requiring primary care.
• Resolve acute issues in the home or clinic and direct to the most effective solutions.
