Job summary

All Seniors Foundation seeks a Geriatrician to provide comprehensive home-based medical care for seniors in Los Angeles. You will diagnose, manage chronic conditions, and guide families through complex healthcare decisions in patients’ homes.

Candidates must be board-certified in geriatrics and licensed in California, with experience in home-based care and strong communication, empathy, and problem-solving skills.

Qualifications

• Board-certified physician with specialization in geriatrics.
• Licensed to practice medicine in California.
• Experience in home-based care models and geriatric syndromes.
• Excellent communication, empathy, and problem-solving abilities.

Responsibilities

• Conduct in-depth medical evaluations and develop personalized care plans.
• Manage chronic conditions, adjust medications, and address acute concerns.
• Collaborate with NPs, PAs, nurses, therapists, and social workers.
• Educate patients and caregivers on disease prevention and health maintenance.
• Document patient encounters and maintain compliance with regulatory standards.
