Job summary

Essen Health Care is seeking a compassionate Nurse Practitioner to provide in-home patient care across the Greater Boston region, Massachusetts. The role focuses on individuals with chronic illnesses, mobility challenges, or limited access to healthcare facilities, delivering holistic care and education in patients' homes.

The NP will perform comprehensive assessments, manage treatment plans, coordinate with physicians, and ensure continuity of care.

Qualifications

• Master’s degree in NP (Family or Adult Ger geriatrics)
• Current Massachusetts NP license
• Bilingual (Spanish) is a plus
• New graduates encouraged to apply
• Experience with EMR/EHR systems preferred

Responsibilities

• Conduct home-based health assessments including exams and med reconciliation
• Diagnose conditions and develop care plans
• Coordinate with physicians and specialists for integrated care
• Educate patients and families on disease management and prevention
• Maintain accurate EHR documentation of visits and follow-ups
• Monitor progress and adjust treatment as needed
• Assess and manage urgent situations during home visits
• Engage with the community to promote health resources
